PREPARE FOR VISIT:
Email, text, fax, or mail back your questionnaire in order to schedule. You can fill them out in word then email, print and fill out then scan, or take a picture of each page with your cellphone to text or email. If you take pictures, please bring the hard copies to your visit. If you need the forms mailed to you, text your mailing address. Please send any abnormal labwork or imaging that is important for your health concern. Don’t worry if you do not have any labwork, can figure out at your first visit if any is necessary if at all.

If you wish to drop off your forms at the office instead of emailing or texting, place them in a sealed envelope and drop them off in the black dropbox to the right of the entrance at the top of the stairs. If the box is wet, please place in the mailbox. Please email or text, so know to pick them up.
If you are getting acupuncture, wear loose fitting comfortable clothes so sleeves and pants can easily be rolled up, or bring clothes to change into. If your health concerns involve the low back, hips, bladder, or reproductive areas, wear low-rise pants.
Many patients are sensitive to perfume and cologne- please consider switching to essential oils for the day. It is better for your health to avoid chemicals in these products.
Click here for directions. Please park in the driveway and NOT along the street, and ring the doorbell at the front door entrance, and wait for someone to answer the door. If garage door needs to be opened to avoid going up and down the stairs to the office, please send a text when you arrive to open the garage door for you.
It is a home office with no waiting room, so please plan accordingly. Dr. Lee gives herself 10-15 minutes between clients to have a snack and use the restroom. Once you enter, please remove your shoes in the foyer before heading downstairs to the office. Please bring socks if you do not like to be barefooted.
The first visit can be up to 1 hour, return visits 45 minutes. First Naturopathic & Acupuncture Combo Visits can be up to 2 hours.
Please cancel 24 hours in advance, as spend at least an hour preparing for your visit. If you do not cancel 24 hours in advance for return visits, you will be charged $30.
All forms of payment are accepted: cash, check, credit card; whatever is your preference. Health savings accounts and flexible spending accounts are accepted. The clinic does not process insurance, but you may try submitting for reimbursement. Tipping is not expected.
Please bring an open mind! It is amazing what can happen when you open yourself to what you think if possible for yourself; without setting a specific expectation or timeline, but focusing on being present with your healing process.
